How old is your test kit? Have you tried using a newer, different brand test kit?
It could just be false positives. If the tank had been running for years, and you're not losing fish, and the fish don't appear stressed, chances are, you're fine.
You're tank is not over crowded, and you have tons of substrate and decors that you should have more than enough bb. So, your filtration should be fine, and not needed to further increase the bb population.
BTW, I also use well water, and I haven't checked my parameters in like 5 years. I've had all my fish for over 5 years, so I just go by their behavior as an indicator of something is off.

Hello; This.The 0.25ppm reading could be ammonium, which is not toxic to fish. Get the Seachem Ammonia Alert to determine if that is free ammonia.
