With that stocking your temps should be no higher than 80. Your fish are suffering from oxygen deficient water...basically slowly suffocating. To make matters worse, the metabolism of fish is sped up at warmer temps, so less oxygen in high temps is deadly. If you don't drop your temps you'll lose your fish

Yesterday, and the day before. Added pimafix also.
Im also planning on changing my media and all pads. What is the best way to do it.
As far as I know Humphead FH is very strong fish hopefully he will make it

Seachem matrix is a good media?
 
Sounds to me like the fish are suffering from lack of oxygen...besides the high nitrates. Water that warm holds very little disolved oxygen. Even discus will suffer if kept at temps 88-91. Lower your temps to at least 80 and bump up your water changes.

I agree. The only reason you should ever have the temp that high is in case of a disease like ich. Even that is temporary. Set the heater to like 80 or pull it completely if your room is very hot. You may have to run a fan over the top if its that hot. Yikes.

Stop medicating, your treating randomly and have no clue what your doing...

Your temps are way too high! Don't change your media like I said before. Simply rinse your existing media and sponges in old tank water in a bucket to get the crap out of it.

Put all the old sponges and media back in the filter except for any ultra fine filter floss type stuff which should be disposed of.

How many gallons is this tank?

180 gallons. I also turned on the lights today. And see very tiny hair looking white worms they moving. Seems planaria . Maybe they are the reason? How to get rid of them?

Your tank seems to be a little on the smaller side for your stock in my opinion. You have cichlids which are some of the dirtiest fish due to how they eat and how often they produce waste. You really have to do more water changes and keep the temp in your tank in the 78 degree range. Make sure you do a good vacuum of the substrate.

I will. You think my Humphead FH will make it ? He is sick and I don't know what's up with him. He is sitting there all day. While other fish seems to act as usual..
